How does Capegard 500 work?
Capecitabine is a fluoropyrimidine chemotherapy given as a tablet. It is a “pro-drug”: the body converts it step by step into fluorouracil (5-FU), and the final step happens mostly inside tumour tissue. 5-FU interferes with the building blocks cancer cells need to copy their DNA, so the cells cannot divide and die.
How long does it take to work? Capecitabine is given in cycles, usually over several months. Your oncology team will judge the benefit with scans and tests.
Capegard 500 dosage: how to take it
- The dose is worked out by your oncologist from your body surface area, so it differs from person to person. A common schedule is twice a day for 14 days, followed by 7 days rest, repeated in 3-week cycles.
- Take the tablets within 30 minutes after a meal, with a glass of water, about 12 hours apart. Swallow them whole.
- Follow the exact number of tablets and the cycle your doctor has written down.
- If you miss a dose, do not make it up. Take the next dose at the usual time and tell your care team.
If you think you have taken too much, contact a doctor or emergency service straight away.
Capegard 500 side effects
Like all chemotherapy, capecitabine can cause side effects, although not everyone gets them.
Common side effects:
- Hand-foot syndrome – redness, swelling, tingling or peeling of the palms and soles
- Diarrhoea, nausea and vomiting
- Mouth sores
- Tiredness and loss of appetite
- Low blood counts, which raise the risk of infection and bruising
Stop taking it and call your care team immediately if you have:
- Diarrhoea 4 or more times a day more than usual, or at night
- Vomiting more than once in 24 hours
- Painful mouth sores that stop you eating or drinking
- Painful redness or blistering of the hands or feet
- A fever of 38°C or higher, or other signs of infection
- Chest pain, or signs of an allergic reaction
Who should not take Capegard 500?
Do not take Capegard 500 if you:
- Are pregnant or breastfeeding
- Are allergic to capecitabine, fluorouracil or any other ingredient of the tablet
- Have a known complete lack of DPD enzyme (dihydropyrimidine dehydrogenase)
- Have severe kidney or liver impairment
- Are taking brivudine or have taken it in the last 4 weeks
Important safety information:
- Some people have reduced DPD activity, which can cause severe side effects. Ask your doctor whether testing is advised.
- Use effective contraception during treatment and for at least 6 months after the last dose.
- Tell your doctor if you have heart disease, kidney or liver problems.
Capegard 500 drug interactions
Tell your doctor about every medicine you take, including those bought without a prescription and herbal products. Important interactions include:
- Warfarin and other coumarin blood thinners – a serious interaction that can cause bleeding; INR must be monitored very closely.
- Brivudine – must never be combined.
- Phenytoin – capecitabine can raise its levels.
- Folinic acid (leucovorin) – can increase capecitabine toxicity.
- Allopurinol – may reduce the effect of capecitabine.
How to store Capegard 500
Store Capegard 500 below 30°C in a dry place, protected from light, in the original blister pack. Keep it out of the sight and reach of children, and do not use it after the expiry date printed on the pack. Ask your pharmacist how to dispose of unused chemotherapy tablets.

Is Capegard 500 the same as Xeloda?
Capegard 500 contains the same active ingredient (capecitabine) at the same strength (500 mg) as Xeloda® 500 mg, so it works in the same way. The difference is the manufacturer, the name, the price and possibly the look of the tablet.
How many capecitabine tablets will I take?
It depends on your body surface area and treatment plan, so the number differs for each person. Your oncologist will write down how many 500 mg tablets to take, twice a day, and for how many days.
How can I prevent hand-foot syndrome?
Use a rich moisturiser on hands and feet, avoid hot water, friction and tight shoes, and tell your care team at the first sign of redness or tingling. They may lower the dose or pause treatment.
Can I take capecitabine with warfarin?
This combination can cause serious bleeding and needs very close INR monitoring, or a switch to another blood thinner. Always tell your oncologist and your anticoagulant clinic.
