How does Imatinib 400 work?
Imatinib is a tyrosine kinase inhibitor, a type of targeted therapy. In CML, an abnormal protein called BCR-ABL tells blood cells to grow without control. In GIST, a protein called KIT does the same. Imatinib blocks these proteins, so the cancer cells stop multiplying and many die, while most healthy cells are left alone.
How long does it take to work? Blood counts often improve within weeks, and your doctor will track your response with regular blood and genetic tests. Imatinib is usually taken long-term.
Imatinib 400 dosage: how to take it
- The usual dose in chronic-phase CML and GIST is 400 mg once a day. In accelerated or blast phase CML, the dose can be 600 mg a day, as decided by your specialist.
- Take the tablet with a meal and a large glass of water to reduce stomach upset.
- Swallow it whole. If you cannot, your doctor may advise dissolving it in water or apple juice.
- Take it at the same time every day and do not skip doses – missed doses can allow the cancer to return.
- If you miss a dose, take it as soon as you remember unless it is nearly time for the next one. Never take a double dose.
If you think you have taken too much, contact a doctor or emergency service straight away.
Imatinib 400 side effects
Like all medicines, imatinib can cause side effects, although not everyone gets them.
Common side effects:
- Fluid retention – puffy eyelids, swollen ankles, weight gain
- Nausea, vomiting and diarrhoea
- Muscle cramps and bone or joint pain
- Skin rash
- Tiredness and headache
- Low blood counts
Get medical help immediately if you have:
- Rapid weight gain, swelling or shortness of breath – possible heart failure or severe fluid retention
- Fever, sore throat or unusual bruising or bleeding
- Yellowing of the skin or eyes, or dark urine – possible liver problems
- A severe skin reaction or signs of an allergic reaction
- Black or bloody stools, or severe stomach pain
Your doctor will check your blood counts, liver function and weight regularly.
Who should not take Imatinib 400?
Do not take imatinib if you:
- Are allergic to imatinib or any other ingredient of the tablet
- Are pregnant or breastfeeding
Important safety information:
- Women who could become pregnant must use effective contraception during treatment and for at least 15 days after the last dose.
- Tell your doctor if you have heart, liver or kidney problems, or have ever had hepatitis B – it can reactivate.
- If you have had your thyroid removed, your thyroid hormone levels need monitoring.
- Imatinib can slow growth in children, so growth should be checked.
- Imatinib can cause dizziness or tiredness – take care when driving.
Imatinib 400 drug interactions
Tell your doctor about every medicine you take, including those bought without a prescription and herbal products. Important interactions include:
- Strong CYP3A4 inhibitors such as ketoconazole, itraconazole, clarithromycin and ritonavir – raise imatinib levels.
- CYP3A4 inducers such as rifampicin, carbamazepine, phenytoin and St John’s wort – lower imatinib levels and may need a higher dose.
- Warfarin – imatinib can interfere with it; low-molecular-weight heparin is usually used instead.
- Paracetamol (acetaminophen) – high or regular doses should be avoided because of liver risk.
- Simvastatin and some other CYP3A4 medicines – imatinib can raise their levels.
How to store Imatinib 400
Store imatinib below 30°C in a dry place, protected from moisture, in the original blister pack. Keep it out of the sight and reach of children, and do not use it after the expiry date printed on the pack.
