Orlistat acts in the gut and very little of it is absorbed into the bloodstream. Because it only affects fat, it has no effect on meals that contain no fat.
How fast does it work? Changes in bowel habits can appear within 1 to 2 days. Weight loss is gradual. Your doctor will usually review your progress after about 12 weeks and may advise stopping if you have not lost at least 5% of your starting weight.
Orlijohn-120 dosage: how to take it
- Take one 120 mg capsule with water three times a day with each main meal that contains fat – during the meal or up to 1 hour after.
- If you miss a meal, or a meal contains no fat, skip that dose.
- Do not take more than three capsules in a day. Taking more does not increase weight loss.
- Follow a nutritionally balanced, reduced-calorie diet, with about 30% of calories from fat spread over three main meals. Meals high in fat make digestive side effects more likely.
- Take a multivitamin containing vitamins A, D, E and K once a day, at bedtime or at least 2 hours after your orlistat dose.
- Always follow the dose and treatment plan your prescribing doctor gives you.

Orlijohn-120 side effects
Like all medicines, orlistat can cause side effects, although not everyone gets them. Most are digestive, are linked to the amount of fat you eat, and often lessen over time.
Common side effects:
- Oily spotting from the back passage
- Wind (gas) with an oily discharge
- Oily or fatty stools
- An urgent need to open your bowels, or opening them more often
- Stomach pain or discomfort
- Headache
- Low blood sugar in people with type 2 diabetes
Stop taking Orlijohn-120 and get medical help if you have:
- Yellowing of the skin or eyes, dark urine, itching or pain in the upper right of your stomach – possible signs of liver problems
- Severe stomach pain spreading to the back – a possible sign of pancreatitis
- Pain in your side or blood in your urine – possible signs of kidney stones
- Bleeding from the back passage
- Signs of an allergic reaction – rash, swelling of the face, lips or throat, or difficulty breathing
Who should not take Orlijohn-120?
Do not take Orlijohn-120 if you:
- Have chronic malabsorption syndrome (difficulty absorbing food from the gut)
- Have cholestasis (a condition that blocks the flow of bile from the liver)
- Are pregnant or breastfeeding
- Are allergic to orlistat or any other ingredient of the capsule
- Are taking ciclosporin, unless your doctor has approved it
Talk to your doctor first if you have diabetes (your diabetes medicine may need adjusting as you lose weight), kidney disease or a history of kidney stones, thyroid problems, epilepsy, or if you take medicines for high blood pressure or cholesterol. Severe diarrhoea may reduce the effect of the contraceptive pill, so use an extra method of contraception if this happens.
Orlijohn-120 drug interactions
Tell your doctor about every medicine you take, including those bought without a prescription. Important interactions with orlistat include:
- Fat-soluble vitamins (A, D, E, K and beta-carotene) – orlistat reduces their absorption; take your multivitamin at least 2 hours apart.
- Ciclosporin – orlistat can lower ciclosporin levels; doses should be taken at least 3 hours apart and levels monitored.
- Levothyroxine – take at least 4 hours apart from orlistat; your thyroid function may need checking.
- Warfarin and other anticoagulants – changes in vitamin K absorption can affect your INR, so more frequent monitoring may be needed.
- Anti-epileptic medicines – tell your doctor if your seizures change.
- HIV medicines and amiodarone – orlistat may reduce their absorption.
